Getting started with PHM

MyPHMCare Pre-registration

An important first step is to complete the pre-registration form to give PHM permission to serve you when you need it. You’ll receive a confirmation email once it’s submitted. A reminder to also complete this form for each family member.
When you complete pre-registration, you’ll have the option to say if you need help now, or if you’re just pre-registering for the future. If you say you need help with a clinical issue now, PHM will contact you within 24 hours to confirm next steps.
